Terms and conditions for Summer Teacher Pass

FIT4MOM PLACER SUMMER TEACHER PASS MEMBERSHIP AGREEMENT

Must be a new member, or have not had a membership in the last 6 months. This pass allows you to attend any classes within the FIT4MOM PLACER franchise between the dates of June 1, 2024 through August 31, 2024.

AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM PLACER to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM PLACER administrator. Charges will be billed from Rose & Jo Fitness, LLC.

No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the FIT4MOM PLACER.

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM PLACER within the initial ten days. You are responsible for paying for the classes already attended.

AGREEMENT
By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
